| June 2005 Client Story When Pam’s 93 year old mother, Emily, returned home from the hospital after her stroke, Pam arranged for private pay care to assist her mom. Pam continued to work. Pam's savings quickly dwindled and she could no longer afford private pay care for Emily. Pam called her local Area Agency on Aging (AAA). AAA staff reviewed Emily’s needs, conducted an assessment, and determined that Emily would be a good candidate for PASSPORT home care. Emily now receives services in the community and remains at home with Pam. Shortly after Emily began receiving PASSPORT home care, Pam’s aunt, Beth, 90, fell and broke her hip. Pam thought her aunt would have to enter a nursing facility because Beth lives alone, but Pam again called the local AAA. AAA staff worked with Beth to determine the best long-term care option for her. Beth does not meet Medicaid financial criteria, but is a woman of moderate income. Beth was able to enter an assisted living facility, where she is able to maintain her independence, but still receive the long-term care she needs. |
