Edna is a 94 year old widow who lives alone in a senior apartment complex. Edna’s only surviving family is an elderly sister. She has been receiving PASSPORT home care for more than five years. Edna suffers from congestive heart failure, arthritis, and is legally blind. Edna requires assistance with bathing, dressing, transfers, and homemaking. Through PASSPORT home care, Edna receives personal care services, home delivered meals, and an emergency response system. Edna’s PASSPORT case manager works with the Society for the Blind to provide Edna with vision-impaired devices and the local senior center for medical transportation. Because of PASSPORT home care and these local community resources, Edna can remain as independent as possible.
